The  Preventing type 2 diabetes presentation is an evidence based program delivered in a group based setting by certified health professionals. The session covers the risk factors, prevention, and strategies to change behaviours. Those at high risk may be eligible to participate in the free and confidential DiabetesVic Life!  telephone health coaching program.

 

Type 2 diabetes

Diabetes is a chronic disease with serious complications, currently affecting an estimated 1.7 million Australians.

Diabetes occurs when the level of glucose (sugar) in the blood becomes higher than normal. Insulin is a hormone that moves glucose from your blood stream, into the cells of your body where it is used for energy. When you have diabetes, the body either can’t make enough insulin or the insulin that is being made does not work properly. This causes your blood glucose level to become too high. High blood glucose levels can affect both your short and long term health.

There are two main types of diabetes – type 1 and type 2. Type 2 diabetes is the most common form of diabetes, affecting 85 to 90 per cent of all people with diabetes. It usually develops in adults over the age of 45, but it is increasingly developing at a younger age. Type 2 diabetes is known as a ‘lifestyle disease’ as it is often triggered by being inactive or carrying excess weight around the abdomen. It tends to run in families and it is not uncommon to have high cholesterol and high blood pressure as well.

 

Preventing type 2 diabetes presentation

The Preventing type 2 diabetes presentation is an evidence based program delivered in a group based setting by certified health professionals. The session covers the risk factors, prevention, and strategies to change behaviours. The prevention session runs for approximately 45 minutes and covers:

  • What is diabetes?
  • The risk factors for type 2 diabetes
  • Free type 2 diabetes risk assessment
  • Tips to reduce your risk
  • Strategies to help you make small but effective lifestyle changes
  • And overview of the extended Life! Program (six month coaching program)
 
The Life! program - free and confidential telephone Health Coaching

The Life! program is available to for people at high risk of developing type 2 diabetes, heart disease, and stroke. The program encourages participants to adopt healthy behaviours and a more active lifestyle. Life! telephone health coaching is a flexible service that fits in with your lifestyle. Telephone health coaching consists of six one-on-one telephone sessions with a trained health professional over a six month period. Your health coach supports you to keep motivated and on track for success.

All participants will receive a comprehensive participant workbook full of practical information on nutrition, physical activity, sleep, stress and how to manage lapses.
